High School: Troy School District students participate in 29 different sports activities. Both Athens and Troy High School have been named one of 10 exemplary athletic programs in the state. All athletics-related forms will need to be signed by both parent/guardian and student in FinalForms for the upcoming school year.
Middle School: All 7th and 8th graders participating in interscholastic athletics in the current/upcoming school year will need to register in FinalForms, where all athletics-related forms will be signed by a parent for the upcoming school year.
Returning parents: simply log in to your account and choose to update for the current/upcoming school year.
MHSAA Requirements for Athletics: Pre-Participation Physicals
An MHSAA pre-participation sports physical is required for all TSD student-athletes who participate in sports. A valid physical examination must be completed on or after April 15 of the current year for the upcoming academic year. A sports physical must be uploaded into FinalForms before the first day of practice/tryouts.
Physicals from the previous academic school year are considered valid for the upcoming academic year provided they were dated April 15 or later.
The Troy School District will be utilizing FinalForms for collection of physical forms for both High School and Middle School athletes. If you have a High School athlete, click on the FinalForms Registration Portal link to set up an account and get your student-athlete registered for the upcoming season.
Locate the parent icon and click NEW ACCOUNT, then follow the steps to create your account.
Type YOUR NAME, DATE OF BIRTH, and EMAIL. Click REGISTER. Note: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder.
Check your email for an ACCOUNT CONFIRMATION EMAIL from FinalForms. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
Click REGISTER STUDENT to register your first child.

ALL student-athletes are required to get a sports physical dated April 15, 2025, or later for the current/upcoming school year. Prior to getting a physical, please update your information in FinalForms, print out the physical form and medical information, and take these documents with you to the doctor. Only the last page, the Preparticipation Physical Evaluation Form, needs to be signed and uploaded to FinalForms.
TSD will again partner with Beaumont Family Medicine to offer athletic physicals for $25 (checks made payable to Beaumont Family Medicine). Any student from our middle or high schools can attend either sports physical date at Athens High School or Troy High.
Guardians must start the FinalForms registration process before student athletes can log into their account and set their password. Both student and guardian are required to complete and sign all forms before students can tryout or start practicing.

Troy School District is using the online system, FinalForms, for families to complete athletics registration for our student athletes. In addition, athletes can register for multiple seasons at the same time!
FinalForms pre-populates information when possible, saving time if you have multiple student athletes and season-to-season. Users may login any time to update information including emergency medical contacts and health information, so that coaches and trainers have the most up to date information.
Suggestion: In Final Forms, select the checkbox for every sport your student is even slightly interested in. This will ensure that you/your student receives information about practices and tryouts, and will allow you to sign all of the required waivers and forms now, rather than having to scramble at the start of each season.
Please Note: Users will be required to sign forms once per year and anytime an update is made.
*Final Athletic Clearance from the TSD Athletic Office is required prior to participating in any sports.

FALL SEASON START DATES
Tryouts for fall sports begin on Monday, August 11 for cross-country, football, girls’ golf, girls’ sideline cheer, boys’ soccer, girls’ swim/dive, boys’ tennis, volleyball, and boys’ water polo. Information about summer workouts and fall season tryout dates and times will be posted on each school’s athletics website:
Pay to Participate Athletic Fees:
Each of the first two sports will cost $200 per sport (must be paid before participating in a scrimmage or game), and the third sport is free. Please do not pay this fee until you have confirmed that your child has made the team. The household max remains at $600. You will be able to make the Pay to Participate payment directly in Final Forms; just follow the prompts.
Pay to Participate Fee Waiver
Students who qualify for free/reduced lunch are eligible for a waiver from the Pay to Participate fee. Parents must complete and submit the current/upcoming application for free/reduced lunch. This form can be found in your child’s main office or on the Food Service webpage.
Parents who check off the waiver box in FinalForms can upload a copy of the verification letter from Food Services to receive the free/reduced fee. Verification forms can also be emailed to Camille Marbury in District Athletics at cmarbury@troy.k12.mi.us. A copy of this verification letter must be received in order to grant the waiver.
Athletic Passes
Athletic passes can be purchased during high school registration in the summer or anytime during the school year: the high school student pass is $60, and adult passes are $80 for one adult or $120 for two adults. Passes can also be purchased via SchoolPay. Please contact the high school athletic office with any questions.
Admission for all Varsity and JV contests taking place in the stadium, gym, and pool will be $7. We will also be utilizing GoFan for electronic ticketing for all Varsity and JV contests.
The four middle schools from which the High Schools draw - Baker, Boulan Park, Larson, and Smith - have also been recognized for their excellence in athletics.

There is no cost for admission to middle school athletic contests.
The MHSAA is a private, not-for-profit corporation of voluntary membership by over 1,500 public and private senior high schools and junior high/middle schools which exists to develop common rules for athletic eligibility and competition. No government funds or tax dollars support the MHSAA, which was the first such association nationally to not accept membership dues or tournament entry fees from schools. Member schools that enforce these rules are permitted to participate in MHSAA tournaments, which attract approximately 1.6 million spectators each year.
The Oakland Activities Association has 24 member high schools located throughout Oakland County, Michigan.
For more than 25 years, the OAA has supported the success of interscholastic athletics and student activities.
Our success includes many MHSAA district, regional and state champions, along with being recognized statewide for our efforts in student leadership.
The purpose of the OAA is to promote interschool relations among the membership of the association and to secure such advantages as may be granted by the union of efforts.
Concussion injuries can occur during sports and other activities and should always be taken seriously. New Michigan Department of Community Health regulations require that we share information with parents and students regarding concussions.
